Appointments
If you have a medical emergency call 911. Do not call the exchange.
Patients are seen by appointment only.
Our office is open to serve you Monday through Friday, from
9:00am to 5:00p.m. Appointments can be made by calling our
office at (818) 344-8822. After 12:00PM (noon) on Fridays the phones
switch over to the exchange for messages, though we are still physically in the office
providing services until 5:00PM. We suggest you call our office approximately
eight weeks before your annual visit is due, so we can schedule
a time that is most convenient for you.If you have a problem or emergency, we will schedule an appointment with Dr. Rebecca Perlow, Dr. Dennis Perlow, or Dr. Tanaz Kahen as soon as possible. *Please keep in mind that Dr. Rebecca Perlow
books annual exams on an average of 3 months in advance.
If you are unable to keep your appointment, please notify
us as soon as possible so we can reschedule your appointment
for another time. If your appointment is before 9:00AM call (818)
344-8822 the morning of your appointment to be sure Dr. Perlow is available to meet with you.
For new patients the following information is required:
- Previous Medical Records (If you think they might be
helpful)
- List of all medications you are presently taking and
supplements.
- Your current medical insurance card and co-payment.
If you have not filled out the forms online we suggest you arrive approximately 15 minutes before your scheduled appointment in order to complete the necessary paperwork.
At the time of your visit, our receptionist will ask you to
complete a patient information form and to sign an authorization
for your insurance company to pay the practice for charges
incurred. Additionally, you may be asked to sign an Arbitration
form, which after reading, you do not have to sign it, you
can simply write “Refused” sign it, date it and
we will continue to see and treat you.
If you are an established patient and have a changed address,
phone number or insurance, we ask that you inform our receptionist
at the time of your appointment so that we may up date your
file.
Please note that we are HIPPA compliant and all information
is strictly confidential.
Laboratory
Each insurance carrier has contracts with different laboratories.
It is the responsibility of the patient to know which laboratory
your insurance carrier has a contract with. We primarily use Quest Diagnostic Laboratory unless you request otherwise, or as required by your insurance carrier.
It is imperative that you inform our medical assistants at
the time of your appointment if other than Quest. We will
not be responsible for any charges should your specimens be
sent to the wrong lab for testing.
Test Results
You will be notified of all abnormal pap results or other abnormal test results by phone.

will contact you to discuss a recommended follow-up plan.
If you can not be reached by phone a message will be left
on a voice recognized machine for you to call our office or
a certified letter will be sent to your current address on
file. We have a test result balance book to be sure tests
ordered have results. If you are worried about your results,
please feel free to call our office.
Prescription and Refills
Routine prescription refills will be made during regular
office hours only. To request a prescription refill, please
have your pharmacy call our office at (818) 344-8822 or fax
the request to (818) 344-3587
Insurance Claims and Payment for Services
Perlow Medical Corporation participates
with most major insurance carriers. Please check with your
insurance company prior to making your appointment to find
out about your coverage.
As a courtesy to our patient, we will file insurance claims
for those insurances with which we participate. We do not
do any billing in the office, our insurance claims are handled
by Practice Financial and they are HIPPA compliant. Please
remember, any amount not covered by insurance is ultimately
the patient’s responsibility. Payment may be made with
cash, check, Mastercharge and Visa credit cards ($15.00 minimum
charge) We do not bill insurance from outside the United States
of America. If you have insurance from a foreign country,
you will pay for all services at the time of the visit and
you will be given a superbill with information needed to be
reimbursed from your insurance carrier.
Co-Payments
All co-payments are to be paid at the time
of your visit, we will not send statements for co-payments.
Payment may be made with cash, or check. (You may use your
Mastercharge or Visa credit card for a $15.00 minimum only)
Obstetrics
If you are an obstetrical patient and you do not have insurance,
we require a $1,000.00 deposit upon your first visit and the
balance to be paid by your 28th week of pregnancy.
Interviews
There is a $300.00 consultation fee charged when meeting with
Dr. Rebecca Perlow. She does not do interviews.
Surgery
If you require surgery; you will be required to meet your
medical deductible (if any) and leave a deposit before any
surgical procedure is performed Perlow Medical Corporations
fees are comparable to the usual and customary fees charged
by other OB-GYN physicians in this area. If you have any questions

Telephone Inquiries
Our Medical office phone number is (818) 344-8822. Our telephone
policy is structured to allow Dr. Rebecca Perlow and Dr. Dennis
Perlow. to care for the patients in
the office with minimum of interruptions. Our front office
staff is available to answer your calls and relay information
to the physician, as appropriate for the physician to return
all calls in between patients or at the end of the working
day. Emergency calls will be handled immediately, while less
urgent calls will be returned as soon as possible.
Emergencies
Dr. Rebecca Perlow is on call 24 hours a day, seven days
a week, in order to handle emergencies and deliveries. If
you have an emergency after hours or on the weekend, please
call our office and the Doctors Exchange will page her. Please
do not call Dr. Perlow for non-emergencies such as routine
prescription refills or appointments. If you have an urgent
